ENROLMENT AGREEMENT
In consideration of my registration/enrolment to the Asian College of Technology International Educational Foundation (ACT-IEF), I hereby certify that the information herein declared are true and correct to the best of my knowledge.
I promise to abide by and comply with the rules and regulations laid down by the school authorities and by the Commission on Higher Education (CHED).
Furthermore, I hereby agree to the following terms and conditions governing my enrolment this current school term:
- That, I am considered officially enrolled only after going through the entire enrollment process (for old and returning student) and complying with the enrolment requirements, i.e., submission of the original Form 138 (for new student), or Certificate of Transfer Credentials (for transferee), and payment of the required enrolment fee/s.
- That, if I withdraw my enrollment before the start of the classes, I shall be charged ten percent (10%) of the total amount due for the entire term, or fifteen percent (15%) if within the first week of classes, or twenty percent (20%) if within the second week of classes, regardless of whether or not I have actually attended classes.
- That, if I withdraw my enrollment at any time after the second week of classes, I shall be charged one hundred percent (100%) of the total amount due for the entire term, regardless of wether or not I have actually attended classes.
- That, if I withdraw my enrollment or drop out of school, I shall forfeit all the school grants and reimburse the school the cost of the school supplies given as part of the enrolment privileges.
- That, in case I am enjoying government subsidy and such subsidy is subsequently forfeited, I shall assume payment of the amount covered by the government subsidy.
- That, I promise to abide by the comply with the rules and regulations promulgated by the competent school authorities and by the Commission on Higher Education.

ACT 100K GRANT FOR COLLEGE STUDENTS TERMS AND CONDITIONS

QUALIFIED STUDENTS
- First time college enrollees in any four-year program are qualified to avail of the ACT 100K Grant in full. The Grant shall be spread over the eight semesters of the program the student is enrolled in at P12,500.00 per semester. Students enrolled in any two-year program can avail a maximum of 50K only, spread over the four semesters of the program enrolled in at P12,500.00 per semester
- For grantees who later decide to shift to another program, they may avail of the Grant beginning in the first semester of their study and can continue to avail of the Grant until the maximum amount (100K/50K) has been redeemed, depending on the duration of the program the student is enrolled in.

Transferees and returnees can avail of the proportionate amount of the grant, provided they have not reached Third Year level in their program of study, as detalled below:
- Transferees and retumees who, upon enrollment, are determined to be in the third semester of their program of study (2nd year-1st semester), can avail a total of P75,000.00 only (P12,500.00 per semester).
- Transferees and returnees who, upon enrollment, are determined to be in the fourth semester of their program of study (2nd year-2nd semester), can avail a total of P62,500.00 only (P12,500.00 per semester).

TERMS AND CONDITIONS

The Grant is applied only to the Tuition component of the total school charges. If the tuition amount is less than P12,500.00, then the grantee can only avail of such amount. The rest of the charges shall be paid in full by the granteeon his/her part.
- The Grant cannot be transferred to another student or converted to cash. Only students in their first or second year of their program may apply for it.
- Crediting of the Grant shall be based on the payments made by the student for his/her counterpart, in equal amount. To illustrate, if a student pays P2,000.00 for his/her counterpart, then he/she is also credited P2,000.00 for the Grant until he/she has covered the P12,500.00. If the student's counterpart is less than P12,500.00, and he/she has paid it in full, then the remaining amount of the grant for the semester shall be credited in full.
- If the student is enjoying other scholarships or subsidies, those scholarships or subsidies shall be applied first. The ACT P100K Grant shall only cover the remaining balance, if any.

The student shall carry a full academic load per semester based on the Prospectus, except for circumstances in which the subject is not offered, or for other reasons external to the student's control or personal circumstances.

The ACT P100k Grant will not cover any subject(s) for which the student re-enrolls in due to Failure in passing the subject(s), No Credit, and/or subject(s) dropped in the previous semester. Thus, the amount of the Grant shall proportionately be decreased.
- If the student carries less load than indicate on the Prospectus for his/her semester of study, but more than or equal to 18 units, the amount of the Grant shall be proportionately decreased

Academic load below 18 units is generally not allowed for the Grant, unless indicated on the Prospectus for the enrolled semester of study. Other exceptions may be granted on a case-to-case basis, as determined by the Scholarship Office.

If the student carries more load than what is indicated on the Prospectus, he/she shall pay for the equivalent charges of the excess loads.

The ACT P100K Grant shall be forfeited for any of the following reasons:
a.
Withdrawal of enrollment;
b.
Dropping out of school;
c.
Non-consecutive semestral enrollment;
d.
Failure to enroll within the early enrollment period set by the school;
e.Failure, no credit (NC), and/or being dropped (DR) in more than two (2) subjects in a semester;
f.
Receiving a weighted grade average below 2.55;
g.
Carrying academic load of less than 18 units
h.
Voluntary transfer to another school;
i.
Being advised to transfer to another school;
k.
Being found guilty of violating any school rules and regulations.

A grantee who has forfeited his/her Grant may be readmitted for valid reasons at the discretion of the Scholarship Office in consultation with the school management.

A grantee shall reimburse the amount granted to him/her for a particular semester, if any of the following apply: he/she drops all the subjects from the previous semester or is given No Credit (NC), or he/she failed to pass all the subjects.
- If a grantee has accumulated an outstanding financial account balance for the last two (2) semesters, the accounting office will issue a waming regarding Forfeiture of the Grant. If the grantee has accumulated an outstanding financial account balance for the last three (3) semesters and fails to settle their account, the Grant will be forfeited
- If, for any reason, the grantee transfers to another school without completing his/her program of study, he/she shall reimburse the total amount previously granted to him/her by way of tuition credit, before the school will release official documents for his/her transfer.
The foregoing terms and conditions may be revised, amended, or changed by the school as deemed necessary and proper.

DATA PRIVACY CONSENT/AGREEMENT
As a student applying for admission/enrolment to Asian College of Technology International Educational Foundation (ACT-IEF), I am giving consent to ACT-IEF to collect, process, retain, and dispose my personal data in relation to the purpose of admission/enrolment to the College.
I am also giving permission to ACT-IEF to post my name as an enrollee in its official website and social media accounts.
I am fully aware that ACT-IEF or its designated representative is duty bound and obligated under the Data Privacy Act of 2012 to protect all my personal and sensitive information that it collects, processes, and retains upon my enrolment and throughout my stay in the College.
Student personal information includes any information about my identity, academics, medical conditions, or any documents containing my identity. This includes but not limited to my name, address, names of my parents or guardians, date of birth, grades, attendance, disciplinary records, and other information necessary for basic administration and instruction.
I understand that my personal information cannot be disclosed without my consent. I understand that the information that was collected and processed relates to my enrolment and to be used by ACT-IEF to pursue its legitimate interests as an educational institution. Likewise, I am fully aware that ACT-IEF may share such information to affiliated or partner organizations as part of its contractual obligations, or with government agencies pursuant to law or legal processes. In this regard, I hereby allow ACT-IEF to collect, process, use and share my personal data in the pursuit of its legitimate interests as an educational institution.
In addition, I am likewise giving my consent/permission in favor of my parents/guardian/representative or whoever is responsible in providing care for me to access, verify, examine and or inspect my academic and scholastic records, school fees/accounts in the College, the result of my physical/medical examination and all matters that relate to my status as a student of the College.
Finally, should I commit any misconduct or should there be a complaint filed against me, before the Student Welfare and Services (SWS) Office by reason of violation of the provisions of the Student Manual or any laws or ordinances, I hereby authorize and give my full consent in favor of the College to inform my parents, guardian, representative or whoever is in charge of providing care or custody for me.

PHOTO/VIDEO CONSENT
I give my consent to Asian College of Technology International Educational Foundation (ACT-IEF) to use my photo or video images in school brochures, advertisements, website, social media account, and in other school publications as the school deems it fit. I hold ACT-IEF, its officers and employees, harmless and free from any liability which may result from the use of my picture or video images for the purposes stated.
This consent or lack thereof, applies throughout my tenure at ACT-IEF and does not need to be updated unless I so desire.
