ENROLMENT AGREEMENT

In consideration of the registration/enrolment of my child/ward at the Asian College of Technology Integrated School Division – Senior High School, I hereby certify that the information herein declared are true and correct to the best of my knowledge.

I promise to abide by and comply with the rules and regulations laid down by competent school authorities and by the Department of Education (DepEd).

Furthermore, I hereby agree to the following terms and conditions on the registration/enrolment of my child/ward for the school term:

  1. That my child is considered officially enrolled only upon submission of all the requirements.
  2. That my child shall forfeit all the school grants and reimburse the school for the supplies given, if I withdraw my child's enrolment, or transfer my child to another school, or if my child drops out of school.
  3. That my child shall enjoy only half of the amount of the school grant if he/she is not grantee of any government subsidy, or if he/she is subsequently disqualified from the SHS Voucher Program or from the ESC Program, provided he/she enrolls during the early enrollment period.
  4. That in case my child forfeits his/her government subsidy, I shall assume payment of the amount covered by the government subsidy.
  5. That if I fail to pay the periodic installment fee, my child may not be allowed to take the periodical examinations.
  6. That, I shall pay Php1,000.00 if I withdraw my child's enrollment before the start of the classes, or Php1,500.00 within the first week of classes, or Php2,500.00 within the second up to the fourth week of the classes, or one hundred percent (100%) of the entire school charges for the school year, after the fourth week of the classes, regardless of whether or not my child has actually attended classes.
  7. That, my child shall be DROPPED from the subject if his/her accumulated absences have reached 20 percent (40 days) or more of the total school days in a year.

DATA PRIVACY CONSENT/AGREEMENT

Being the parent/guardian of the above named student who is a minor and applying for admission/enrolment into Asian College of Technology Integrated School Division (ACT-ISD), I am giving consent to ACT-IEF to collect, process, retain, and dispose my child’s/ward’s personal data in relation to the purpose of admission/enrolment into the school.

I am also giving permission to ACT-ISD to post the name of my child/ward as an enrollee in its official website and social media accounts.

I am fully aware that ACT-ISD or its designated representative is duty bound and obligated under the Data Privacy Act of 2012 to protect my child’s/ward’s personal and sensitive information that it collects, processes, and retains upon my child’s/ward’s enrolment and throughout my child’s/ward’s stay in the school.

Student personal information includes any information about my child’s/ward’s identity, academics, medical conditions, or any documents containing my child’s/ward’s identity. This includes but not limited to my child’s/ ward’s name, address, names of parents or guardians, date of birth, grades, attendance, disciplinary records, and other information necessary for basic administration and instruction.

I understand that my child’s/ward’s personal information cannot be disclosed without my consent. I understand that the information that was collected and processed relates to my child’s/ward’s enrolment and to be used by ACT-IEF to pursue its legitimate interests as an educational institution. Likewise, I am fully aware that ACT-IEF may share such information to affiliated or partner organizations as part of its contractual obligations, or with government agencies pursuant to law or legal processes. In this regard, I hereby allow ACT-IEF to collect, process, use and share my child’s/ward’s personal data in the pursuit of its legitimate interests as an educational institution.

PHOTO/VIDEO CONSENT

Being the parent/guardian of the above named student who is a minor, I give my consent to Asian College of Technology Integrated School Division (ACT-ISD) to use the photo or video images of my child/ward in school brochures, advertisements, website, social media accounts, and in other school publications as the school deems it fit. I hold ACT-ISD, its officers and employees, harmless and free from any liability which may result from the use of said picture or video images for the purposes stated.

This consent or lack thereof, applies throughout my child’s/ward’s tenure at ACT-ISD and does not need to be updated unless I so desire.
